Attached or enclosed are the following supporting documents from the Applicant, to be used at the discretion of the Scholarship Committee in determining status of application.

1. A copy of the applicant’s most recent Academic Marks which should be in the form of a Statement from the Department of Education or “Transcript of Secondary Level Achievement”

2. An outline of any and all academic awards received and achievements bestowed upon the applicant by their school or educational environment.

3. A brief history of the applicant’s participation in hockey in general, with a specific outline of participation within the North Sask Female Hockey League.

4. An outline containing any awards or achievements that the applicant has received throughout their hockey career to-date.

5. A brief history of activities in the community and/or school that the applicant has participated in, and the role that the applicant played in each. Please ensure to include all activities.

6. Two ( 2 ) letters of reference.
